Affidavit (Clearance)

AFFIDAVIT

I, JACK SPARROW, of legal age, Filipino, with office address at the 1234 Wonderland Bldg, 1234 Roxas Boulevard, Manila, duly sworn to according to law, depose and state that:

a) I am the President and CEO of Sparrows Training Center (hereinafter the “Company”), a corporation organized and existing under Philippine laws with principal office located at the 1234 Wonderland Bldg, 1234 Roxas Boulevard,Manila.

b) The lease contract of the Company with Wonderland Building which expires on September 19, 2018 will be renewed under new terms that will be negotiated before the expiry date.

c) This affidavit is executed to attest to the Company’s compliance with equirements for its SSS Clearance.
